Over the years our services have been developed by listening carefully to feedback from service users, employees, families and other professionals. Your Care Team works in partnership with service users and families to design care plans that aid development, promote independence and support our service users. Our care plans not only develop our service users' skills but support them to reach their personal goals and aspirations.
We offer our services for all vulnerable adults throughout Kirklees and Bradford, in their own homes, community environments and our Your Care Team bases. Our staff are friendly, professional, experienced and willing to go above and beyond for our service users. All Your Care Team employees undergo a rigorous recruitment process including an Enhanced DBS check.
All of the services offered by Your Care Team Ltd are approved and regulated by the Care Quality Commission.

We offer a range of services to help our service users and their families to live their lifestyle at ease and as independently as possible. Your Care Team believes that every single individual is unique, with their own individual needs and preferences.
Your Care Team is dedicated to working in partnership with service users and there families to find solutions, which promote independence, support development and benefit our service users by improving their quality of life. Our qualified and knowledgeable assessors work with service users and families/guardians to design tailor made care plans. Which inform Your Care Team employees of what support is necessary and how it should be provided.
The variety of domiciliary services Your Care Team offers includes:
- Domestic assistance including cleaning, laundry, ironing, gardening and shopping.
- Support to use local facilities, including shops, banks, post offices, appointments andlibraries.
- Assisting, prompting or administering medication.
- General support service which enables families to flexibly choose any necessary tasks the support worker undertakes to meet the service users’ needs.
- Preparation and planning of meals, including assistance with eating and drinking.
- Assistance with personal care tasks, including bathing, showering, washing and toilet needs.
Cleaning, Laundry & Shopping
Your Care Team offers a range of cleaning services including: shopping, gardening, cleaning toilets and bathrooms etc. We understand that how you want your domestic tasks to be carried out, so these services can be tailored to our service users’ needs.
Sitting Service (Adult)
Your Care Team's service is designed to support service users' families/carers, so that they can have a hard-earned break from their caring responsibilities and give them time to do their own tasks which they haven't been able to do. As well as giving them the opportunity to pursue their hobbies and just relax. We offer a variety of flexible sitting services that can be provided day, evening or night from 8am to Midnight.
Out & About in the Community
Your Care Team offers a variety of opportunities that enable service users to fully play a part in their communities. We support our service users' to discover the range of activities available, so they can choose which ones they feel will best aid their development and effectively contribute to meeting their personal goals and aspirations. We work with service users to conclude the type and level of support they require to safely and effectively engage in these activities.
The main aim of this service is to aid development and maximise independence and control over their lives in a supportive manner, which develops those crucial key life skills. Some of the services we offer include: tailored support to access educational, social, leisure; work related activities or community facilities and organised group outings/trips e.g. cinema, restaurants, shopping etc.
Supported Living Services
The supported living services which Your Care Team offers allows you to live life on your own terms and how you want. This service is flexible and is catered to your individual needs by listening and getting to know you. Our staff at Your Care Team will put together a unique package of support in place with a carefully chosen carer. We will help you stay within the comfort of your own home that you identify with and adore. Since, it is increasingly becoming more common for families to choose home care rather than choosing to be placed within a residential care home. We are dedicated to ensuring we provide ongoing support to you and your carefully selected carer throughout your unique care package.
The daytime respite care provided by Your Care Team Ltd is a temporary care arrangement, which offers rest or relief to primary carers. Your Care Team Ltd understands that caring for a vulnerable adult can be physically and mentally demanding. We understand it is much more than handling everyday tasks. Particularly, when caring for long periods at time which can be stressful as well as emotionally draining.
Our daytime respite care provides your main carer a well-earned break while still ensuring your care needs are met. Since, Your Care Team can offer extra support for a few hours each day or each week depending on what your individual needs require. We will ensure that the vulnerable adult that you are supporting will always have someone there to help them whenever for whatever reason you are absent.
Day Care Opportunities for Adults with Learning Disabilities
Your Care Team Ltd are a registered provider of leisure and learning opportunities for vulnerable adults. We work with vulnerable adults to achieve a more independent life and improve the quality of their lives by empowering them to accept their right to social inclusion. Also, we understand that everyone has different needs and concerns, so we ensure our staff are well equipped to engage our service users in activities which will aid there personal development and work towards meeting their individual goals.
Also, this day-care opportunity service provided by Your Care Team is available on a flexible basis. Since, in addition to the regular day-care opportunity service we provide there is an additional 'Drop in' Day-care Opportunities for Adults with Learning Disabilities' available. For those individuals who are unsure of how frequently they would like to utilise this service.
